Long term opiate addiction how to taper down ... or suggest a way to explain to the doctor that I want off.?

Hello all my name is chris and im new to this forum/disscussion board...here is my deal I have had kidney problems for going on 8 years now my right kidney is crystalizing and I have massive like 6.5mm stones that I pass just about everyother day.. mind you this has been going on 8 years... I am highly dependent on opiates... because simply put not a damn thing else works.... they first started me out on Norco 7.5 5x and day for 3 years ...then to the 10mg 5x daily ... well about 3 years they stopped working all together so I was put on percocet 10/325 6 tablets daily as needed...totally i was on hydrocodone for 5 years and oxy for 3... well 2 years ago the percocet stopped working and he switched me to roxicodone 15mg 1 pill 7x a day and i have been on that since....I am in severe pain but at this point I am so physically addicted that I am tired of being on the damn things... i don't abuse them i have always taken them as directed... but I know that withdrawal is HELL especially the length of time i have been taken them and the dose escalation...I do not know how to bring this up to my doc...I have 2 of them that work on me in the same medical building... and they both have been excellent ... i know you can taper but i dont know how to exactly... just any info would be appreciated ... thanks guys.
